Ezetimibe, Atorvastatin falls under GLUCURONIDATED DRUGS — the group Milk Thistle interacts with.

Theoretically, milk thistle might affect the clearance of drugs that undergo glucuronidation.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.

Here's the gist with Liptruzet, which combines ezetimibe and atorvastatin. Ezetimibe is processed in the body largely through a step called glucuronidation, and lab studies hint that milk thistle's silymarin might slow that enzyme down. In theory, that could let a bit more of the drug linger, which might nudge up the chance of side effects like muscle aches, stomach upset, or, rarely, liver enzyme changes.

I want to be honest, though: this is only seen in test tube and animal research, not in real patients, so the real-world risk looks small. Culinary or occasional use is unlikely to be an issue. Still, since both ingredients touch the liver, it's worth running it by your pharmacist or doctor before pairing them regularly.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

The atorvastatin part of Liptruzet is cleared by a liver enzyme called CYP3A4. In theory, if milk thistle slowed that enzyme down, more atorvastatin could linger in your body, which might nudge up the risk of statin side effects like muscle aches, weakness, or rarely liver irritation. The good news is that the evidence here is weak. Lab studies disagree with each other, and human studies using milk thistle doses around 420 to 1350 mg daily did not meaningfully change how the body handles other CYP3A4 medicines.

So for most people this is an unlikely, minor concern. Ezetimibe isn't really affected this way. Still, if you notice new muscle pain or unusual tiredness, or you want to start milk thistle, just run it by your pharmacist or doctor first.

HMG-CoA REDUCTASE INHIBITORS ("Statins")

Ezetimibe, Atorvastatin also falls under HMG-CoA REDUCTASE INHIBITORS ("Statins").

Theoretically, milk thistle might interfere with statin therapy by decreasing the activity of organic anion transporting polypeptide 1B1 (OATB1B1) and inhibiting breast cancer resistance protein (BCRP).
Preliminary evidence suggests that a milk thistle extract of silymarin can decrease the activity of the OATP1B1, which transports HMG-CoA reductase inhibitors into the liver to their site of action, and animal research shows this increases the maximum plasma concentration of pitavastatin and pravastatin. The silibinin component also inhibits BCRP, which transports statins from the liver into the bile for excretion. However, in a preliminary study in healthy males, silymarin 140 mg three times daily had no effect on the pharmacokinetics of a single 10 mg dose of rosuvastatin.

Here's the gist with Liptruzet, which combines atorvastatin (a statin) and ezetimibe. The theory is that milk thistle's silymarin may slow down certain transporters that move statins into and out of the liver. If that happened, a bit more atorvastatin could linger in your blood, which in theory might raise the odds of statin side effects like muscle aches, soreness, or weakness.

That said, I want to keep this in perspective. This is rated a minor and unlikely interaction, and the evidence is weak, mostly lab and animal work, with one human study actually showing no effect on a similar statin. So for most folks this is more of a footnote than a worry. Still, it's worth a quick mention to your pharmacist or doctor, and let them know if you notice any new muscle pain.

Ezetimibe, Atorvastatin also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).

Milk thistle may inhibit one form of OATP, OATP-B1, which could reduce the bioavailability and clinical effects of OATP-B1 substrates.
In vitro research shows that milk thistle inhibits OATP-B1. Two case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are OATP substrates, including sorafenib and methotrexate. OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds into the body. Inhibition of OATP may reduce the bioavailability of oral drugs that are substrates of OATP.

Here's the gist with Liptruzet (atorvastatin plus ezetimibe) and milk thistle. The atorvastatin part relies on a liver transporter called OATP-B1 to get pulled into your liver cells, which is exactly where it does its cholesterol-lowering work. In test-tube studies, milk thistle can block that transporter a little. In theory, that could nudge how much of the drug reaches its target.

The honest answer is that this is very weak, lab-only evidence, and it's rated minor. I wouldn't expect any real change in how well your cholesterol numbers improve or in side effects like muscle aches. Still, since this combo is so widely used, it's worth a quick mention to your pharmacist or doctor, especially if you take a concentrated milk thistle supplement daily.
