Esterified Estrogens, Methyltestosterone falls under GLUCURONIDATED DRUGS — the group Milk Thistle interacts with.

Theoretically, milk thistle might affect the clearance of drugs that undergo glucuronidation.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.

Here's the gist with Estratest (esterified estrogens plus methyltestosterone) and milk thistle. Your body clears these hormones partly by tacking on a sugar molecule, a process called glucuronidation. In lab and animal experiments, milk thistle's silymarin slowed that very step. In theory, that could let estrogen and testosterone levels build up a little higher than expected.

If that happened, you might notice more of the usual hormone-related effects: breast tenderness, bloating, mood changes, or for the testosterone side, acne or unwanted hair changes. But honestly, this is theoretical and only seen in test tubes, not in real people, so I wouldn't lose sleep over it. Just give your pharmacist or doctor a heads-up before pairing them so we can keep an eye out.

Here's the thing to keep in mind with Estratest, which combines esterified estrogens with methyltestosterone. The concern with milk thistle isn't about how your body breaks the medicine down. Instead, lab and animal studies hint that silymarin (milk thistle's active part) might gently attach to estrogen receptors, the same docking spots your estrogen works on. In theory, that could slightly nudge against the estrogen part of your therapy.

But I want to be honest about how thin this evidence is. It comes only from test tube and animal work, the interaction is rated minor, and we don't have human data showing it actually weakens your treatment or changes symptoms like hot flashes. I wouldn't lose sleep over it. Still, it's worth a quick mention to your pharmacist or doctor before adding milk thistle.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Estratest combines an estrogen with methyltestosterone, and both of those hormones are partly broken down by a liver enzyme called CYP3A4. The thinking here is that if milk thistle slowed that enzyme down, your body might clear these hormones a little more slowly, which in theory could nudge up hormone-related effects like breast tenderness, bloating, nausea, mood changes, or acne.

The good news: this one is rated as minor and unlikely, and the evidence is weak. Careful human studies actually found milk thistle did not meaningfully change how the body handles other CYP3A4 drugs. So for most people this combo is not a real worry. Still, since hormone therapy carries its own risks, it is worth a quick chat with your pharmacist or doctor before adding milk thistle.
