Why this applies to Cabotegravir

Cabotegravir falls under GLUCURONIDATED DRUGS — the group Breadfruit interacts with.

Breadfruit might increase the levels of drugs metabolized by uridine diphosphoglucuronosyl transferase (UGT). So far, this interaction has not been reported in humans. However, in vitro evidence shows that the breadfruit constituent artocarpin inhibits UGT. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Some drugs that are metabolized through glucuronide conjugation include acetaminophen (Tylenol, others) and oxazepam (Serax), haloperidol (Haldol), lamotrigine (Lamictal), morphine (MS Contin, Roxanol), zidovudine (AZT, Retrovir), and others.

Here's the connection: your body clears cabotegravir mainly through a process called glucuronidation, run by an enzyme family known as UGT. Breadfruit contains a compound (artocarpin) that, in lab studies, can slow that same enzyme down. In theory, if cabotegravir is cleared more slowly, its levels could creep up, which might mean more chance of side effects like headache, fever, fatigue, or reactions at the injection site.

I want to be straight with you, though: this is only test-tube evidence, and it has never been shown to happen in real people. Eating breadfruit as a food is very unlikely to matter. Concentrated leaf teas or extracts are the bigger unknown. Because keeping HIV medication steady is so important, please run any breadfruit supplement past your pharmacist or doctor first.

About Cabotegravir

What Is Cabotegravir Used For?

Brand name: Apretude

Vocabria (cabotegravir) tablets are used in combination with rilpivirine (Edurant) to treat HIV-1 infection in adults and adolescents aged 12 and older who weigh at least 35 kg and whose HIV is already virologically suppressed (virus levels undetectable or very low on their current regimen). Vocabria tablets serve as a short-term oral lead-in — taken for roughly one month — before switching to long-acting injectable therapy.

Cabotegravir tablets are also used as an oral lead-in for HIV-1 pre-exposure prophylaxis (PrEP) — helping HIV-negative adults and adolescents (weighing at least 35 kg) confirm they tolerate the drug before starting injectable PrEP. Both uses require confirmed HIV-negative status before starting.
