Why this applies to Carbamazepine

Carbamazepine falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES — the group Breadfruit interacts with.

Breadfruit might increase the levels of drugs metabolized by cytochrome P450 2C8 (CYP2C8) enzymes. So far, this interaction has not been reported in humans. However, in vitro evidence shows that the breadfruit constituent artocarpin inhibits CYP2C8 enzymes, which might increase the levels of CYP2C8 substrates. Some drugs metabolized by CYP2C8 include amiodarone (Cardarone), paclitaxel (Taxol); nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Voltaren) and ibuprofen (Motrin), rosiglitazone (Avandia), and others.

Here's the gist for your situation: a compound in breadfruit called artocarpin has been shown, in a test tube only, to slow down a liver enzyme known as CYP2C8. In theory, blocking that enzyme could let a drug it handles build up a bit higher than usual. With carbamazepine, if its levels crept up, you might notice things like dizziness, drowsiness, blurry or double vision, or unsteadiness on your feet.

That said, I don't want to overstate this. There are no human reports of this actually happening, and carbamazepine is mostly cleared by a different enzyme (CYP3A4). Eating breadfruit as food is very unlikely to matter. Concentrated leaf teas or extracts are the bigger question mark, so check with your pharmacist before using those.

Carbamazepine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the gist for carbamazepine (Tegretol): your body relies heavily on a liver enzyme called CYP3A4 to break this medicine down. Lab and animal studies hint that compounds in breadfruit leaves may gently slow that same enzyme. If that happened in people, carbamazepine could clear a bit slower and drift higher, which might mean more dizziness, drowsiness, blurred or double vision, or unsteadiness.

Honestly though, this is mostly theoretical right now, based on test-tube and animal work, not real patients. Eating breadfruit as food is very unlikely to matter. Concentrated leaf teas or extracts are the bigger question mark. Since carbamazepine needs careful, steady levels, just check with your pharmacist or doctor before using any medicinal breadfruit product.

What Is Carbamazepine Used For?

Brand names: Tegretol, Tegretol XR, Equetro, Carbatrol, Carnexiv

Carbamazepine is used to treat certain types of seizures — including partial seizures, generalized tonic-clonic (grand mal) seizures, and mixed seizure patterns. It works best for those specific seizure types; it does not appear to control absence (petit mal) seizures.

It is also prescribed to relieve the intense facial pain of trigeminal neuralgia — a nerve condition that causes sudden, severe pain along the face. It is not a general painkiller and shouldn't be used for ordinary aches and pains.
