Verapamil falls under ANTIHYPERTENSIVE DRUGS — the group Breadfruit interacts with.

Theoretically, concomitant use with antihypertensive drugs might increase the risk of hypotension. Animal research shows that breadfruit leaf extract can decrease heart rate and weaken the force of heart muscle contractions, which may lower blood pressure.
Some antihypertensive drugs include captopril (Capoten), enalapril (Vasotec), losartan (Cozaar), valsartan (Diovan), diltiazem (Cardizem), amlodipine (Norvasc), hydrochlorothiazide (HydroDIURIL), furosemide (Lasix), and many others.

Here's the gist with breadfruit and your verapamil. Verapamil already lowers blood pressure and slows your heart rate. In animal studies, breadfruit leaf extract seemed to do something similar, gently easing the heart's rate and the strength of its squeeze. So if you pair concentrated breadfruit leaf teas or extracts with verapamil, the two could theoretically add up and push your blood pressure or heart rate lower than you want.

What that might feel like is dizziness, lightheadedness when you stand, unusual tiredness, or a slow pulse. I'll be honest though: this is based on lab and animal work, not human studies, so it's a possibility rather than a proven problem. Eating breadfruit as a regular food is unlikely to matter. Before using any breadfruit leaf product, just run it by your pharmacist or doctor first.

CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES

Verapamil also falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES.

Breadfruit might increase the levels of drugs metabolized by cytochrome P450 2C8 (CYP2C8) enzymes. So far, this interaction has not been reported in humans. However, in vitro evidence shows that the breadfruit constituent artocarpin inhibits CYP2C8 enzymes, which might increase the levels of CYP2C8 substrates. Some drugs metabolized by CYP2C8 include amiodarone (Cardarone), paclitaxel (Taxol); nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Voltaren) and ibuprofen (Motrin), rosiglitazone (Avandia), and others.

Here's the gist with breadfruit and verapamil. A compound in breadfruit leaves called artocarpin appears to block a liver enzyme (CYP2C8) in lab testing. If that same blocking happened in your body, your verapamil could clear out more slowly and build up a bit. Since verapamil already slows the heart and relaxes blood vessels, higher levels could nudge those effects too far, think extra-low blood pressure, dizziness, a slow pulse, tiredness, or ankle swelling and constipation.

The reassuring part: this is only seen in test-tube studies, never reported in real people, and eating breadfruit as a food is unlikely to matter. Concentrated leaf teas or extracts are the bigger question mark. Before using any breadfruit product medicinally, check with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Verapamil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Your body relies on a liver enzyme called CYP3A4 to break verapamil down. There's some early lab and animal work suggesting compounds in breadfruit (like one called artocarpin) might slow that enzyme a bit. If that happened in people, verapamil could clear more slowly and build up, which in theory could nudge its effects up: lower blood pressure, a slower heart rate, dizziness, constipation, or ankle swelling.

Here's the honest part: this is only a theoretical, possible concern based on test-tube and animal studies, not real human data, and breadfruit's effect looked weak.
