Why this applies to Diazepam

Diazepam falls under GLUCURONIDATED DRUGS — the group Breadfruit interacts with.

Breadfruit might increase the levels of drugs metabolized by uridine diphosphoglucuronosyl transferase (UGT). So far, this interaction has not been reported in humans. However, in vitro evidence shows that the breadfruit constituent artocarpin inhibits UGT. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Some drugs that are metabolized through glucuronide conjugation include acetaminophen (Tylenol, others) and oxazepam (Serax), haloperidol (Haldol), lamotrigine (Lamictal), morphine (MS Contin, Roxanol), zidovudine (AZT, Retrovir), and others.

Here's the short version: a compound in breadfruit leaves called artocarpin has been shown, in lab dishes only, to slow down a liver pathway called UGT (glucuronidation). Some of the steps that clear diazepam (Valium) from your body run through this pathway. In theory, if breadfruit leaf extracts blocked it, diazepam could hang around longer and build up, which might mean more drowsiness, dizziness, foggy thinking, or unsteadiness than usual.

That said, I want to keep this in perspective. This has never been seen in real people, and it comes only from test-tube work. Eating breadfruit as a food is very unlikely to matter. Concentrated leaf teas or supplements are the bigger unknown. If you take Valium and want to try breadfruit products medicinally, just run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Diazepam also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the gist: your body uses an enzyme called CYP3A4 to help break down diazepam (Valium). Lab and animal studies hint that compounds in breadfruit leaves might slow that enzyme down a little. If it did, diazepam could clear more slowly and build up, which in theory might make you feel extra drowsy, foggy, dizzy, or unsteady on your feet.

The good news is this is only a theoretical, weak signal from test-tube and animal research, and no human studies show it actually matters. Eating breadfruit as food is very unlikely to cause any problem. Concentrated leaf teas or extracts are the bigger question mark. If you're using a breadfruit product for a health reason, just run it by your pharmacist or doctor first.

About Diazepam

What Is Diazepam Used For?

Brand names: Valium, Valium injection, Diastat AcuDial, Valtoco

Diazepam treats several different problems, depending on the product. Oral diazepam (tablets such as Valium, and oral solution) is used for anxiety disorders, short-term relief of anxiety symptoms, and symptom relief in acute alcohol withdrawal. It is also used alongside other treatments for muscle spasm and, adjunctively, for convulsive disorders (seizure disorders).

Diazepam injection is used for the same anxiety and alcohol withdrawal uses. It is also used before endoscopic procedures, before surgery, and before cardioversion (when given intravenously). It helps with muscle spasm and tetanus, and with status epilepticus (a prolonged, dangerous seizure) as an add-on. Diazepam rectal gel and Valtoco nasal spray are used only for seizure clusters (acute repetitive seizures) in people with epilepsy ages 2 and older.
