Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine falls under GLUCURONIDATED DRUGS — the group Breadfruit interacts with.

Breadfruit might increase the levels of drugs metabolized by uridine diphosphoglucuronosyl transferase (UGT). So far, this interaction has not been reported in humans. However, in vitro evidence shows that the breadfruit constituent artocarpin inhibits UGT. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Some drugs that are metabolized through glucuronide conjugation include acetaminophen (Tylenol, others) and oxazepam (Serax), haloperidol (Haldol), lamotrigine (Lamictal), morphine (MS Contin, Roxanol), zidovudine (AZT, Retrovir), and others.

This combination product contains acetaminophen, and that's the piece that matters here. Acetaminophen is partly cleared by an enzyme system called UGT (glucuronidation). A compound found in breadfruit leaves, called artocarpin, has slowed down that enzyme in a test tube. In theory, if breadfruit blocked UGT in your body, acetaminophen could hang around longer and build up, which raises the worry of liver stress, especially at higher doses.

The honest part: this has never actually been seen in people. It comes only from lab studies, so it's a theoretical concern. Eating breadfruit as food is very unlikely to cause trouble, and concentrated leaf teas or extracts are the bigger unknown. Just keep your acetaminophen within recommended limits, and check with your pharmacist or doctor before using breadfruit medicinally.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

This combination product (Hycomine Compound) packs several ingredients, and the one worth focusing on here is the hydrocodone, an opioid pain reliever. Your liver uses an enzyme called CYP3A4 to help process it. In lab dishes and animal studies, a breadfruit compound (artocarpin) was found to weakly slow that same enzyme. If that happened in people, hydrocodone could linger a bit longer than usual, which in theory might bump up drowsiness, dizziness, constipation, or slowed breathing.

That said, this is very weak, theoretical evidence, and eating breadfruit as food is unlikely to cause any real problem. Concentrated leaf teas or extracts are the bigger unknown. If you plan to use those medicinally, just check with your pharmacist or doctor first.
