Why this applies to Doxorubicin

Doxorubicin falls under GLUCURONIDATED DRUGS — the group Breadfruit interacts with.

Breadfruit might increase the levels of drugs metabolized by uridine diphosphoglucuronosyl transferase (UGT). So far, this interaction has not been reported in humans. However, in vitro evidence shows that the breadfruit constituent artocarpin inhibits UGT. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Some drugs that are metabolized through glucuronide conjugation include acetaminophen (Tylenol, others) and oxazepam (Serax), haloperidol (Haldol), lamotrigine (Lamictal), morphine (MS Contin, Roxanol), zidovudine (AZT, Retrovir), and others.

Here is the short version on mixing breadfruit with doxorubicin. Your body partly clears doxorubicin by attaching a sugar molecule to it, a process handled by enzymes called UGTs. A compound in breadfruit leaves (artocarpin) appears to slow those enzymes down, at least in lab dish studies. In theory, if that clearance route is blocked, more doxorubicin could linger in your system, which might mean stronger effects and a higher chance of side effects like mouth sores, nausea, low blood counts, or the heart strain this drug is already watched for.

That said, this is only laboratory evidence and has not been seen in real patients, so don't panic. Eating breadfruit as a food is very different from taking concentrated leaf extracts or teas. Since chemotherapy is serious business, please run any breadfruit supplements by your oncology team or pharmacist first.

Doxorubicin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the gist: your body partly uses an enzyme called CYP3A4 to help clear doxorubicin. Lab and animal studies hint that compounds in breadfruit (like artocarpin) might mildly slow this enzyme down. In theory, that could let a bit more doxorubicin linger in your system, which could nudge up its well-known side effects like nausea, mouth sores, low blood counts, hair loss, or heart strain.

Now, the honest part: this is based only on test-tube and animal research, and it's rated a minor concern. Eating breadfruit as a normal food is very unlikely to matter. Concentrated leaf teas or extracts are the bigger unknown. Since chemo is serious business, please run any breadfruit supplement past your oncologist or pharmacist first.

Doxorubicin is used to treat a wide range of cancers. It is given as part of combination chemotherapy for women with breast cancer that has spread to the lymph nodes after surgery, and it is used for many other cancers — from blood cancers like leukemia and lymphoma to solid tumors like bone and soft tissue sarcomas.

It is considered a broad-spectrum cancer fighter, meaning it is active against many different tumor types. It is almost always given alongside other chemotherapy drugs rather than on its own.
