Why this applies to Acetaminophen, Aspirin, Caffeine

Acetaminophen, Aspirin, Caffeine falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Breadfruit interacts with.

Theoretically, concomitant use with anticoagulant/antiplatelet drugs might increase the risk of bleeding. In vitro research shows that certain flavonoid constituents from breadfruit root extract can inhibit thromboxane formation and reduce platelet aggregation.
Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's the piece that matters in your Excedrin: the aspirin. Aspirin already makes your platelets less sticky, which is part of how it eases pain and thins the blood a bit. Lab studies suggest some compounds in breadfruit may do something similar, nudging platelets to clump less. Stack the two together and, in theory, you could bruise more easily or be slower to stop bleeding from a small cut. The acetaminophen and caffeine parts aren't really the concern here.

I want to be honest about how soft this evidence is. It comes only from test-tube work using concentrated breadfruit extracts, not from people, and breadfruit eaten as food is very unlikely to matter. If you use breadfruit leaf teas or supplements regularly, just mention it to your pharmacist or doctor.

GLUCURONIDATED DRUGS

Breadfruit might increase the levels of drugs metabolized by uridine diphosphoglucuronosyl transferase (UGT). So far, this interaction has not been reported in humans. However, in vitro evidence shows that the breadfruit constituent artocarpin inhibits UGT. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Some drugs that are metabolized through glucuronide conjugation include acetaminophen (Tylenol, others) and oxazepam (Serax), haloperidol (Haldol), lamotrigine (Lamictal), morphine (MS Contin, Roxanol), zidovudine (AZT, Retrovir), and others.

If you're reaching for an Excedrin product (which combines acetaminophen, aspirin, and caffeine), here's the piece that's worth knowing. The acetaminophen part gets cleared from your body by a process called glucuronidation. There's some early lab work suggesting a compound in breadfruit, called artocarpin, can slow that pathway down. In theory, that could let acetaminophen build up a bit more than usual, and acetaminophen in higher amounts is hard on the liver.

I want to be honest about how weak this evidence is. It comes only from test-tube and animal studies, and it has never actually been seen in people. Eating breadfruit as a food is very unlikely to matter. If you're using concentrated breadfruit leaf teas or extracts regularly while taking these products, just check in with your pharmacist first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Acetaminophen, Aspirin, Caffeine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the practical picture for your Excedrin (the acetaminophen, aspirin, and caffeine combo). Breadfruit, mainly its leaf extracts, has shown a weak ability to slow down a liver enzyme called CYP3A4 in lab dishes and animal studies. In theory, blocking that enzyme could let certain drugs build up a bit. But honestly, none of these three ingredients lean heavily on CYP3A4, so this concern is mostly hypothetical here.

If anything did shift, you might notice slightly stronger effects like stomach upset from the aspirin or extra jitteriness from the caffeine, but that's unlikely. Eating breadfruit as food is no worry at all. If you're using concentrated leaf teas or extracts regularly, just mention it to your pharmacist or doctor to be safe.
