Breadfruit and Effient

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Breadfruit interacts with whole categories of medication. Effient falls into 2 of those categories, so it can interact with Breadfruit in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ANTICOAGULANT/ANTIPLATELET DRUGS

1 of 2

Why this applies to Effient

Effient falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Breadfruit interacts with.

Theoretically, concomitant use with anticoagulant/antiplatelet drugs might increase the risk of bleeding. In vitro research shows that certain flavonoid constituents from breadfruit root extract can inhibit thromboxane formation and reduce platelet aggregation.
Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the key thing about Effient (prasugrel): it's already a powerful blood thinner that stops platelets from sticking together, and you're usually taking it with aspirin too. The concern with breadfruit isn't about how Effient is broken down. Instead, lab studies hint that certain flavonoid compounds in breadfruit may also reduce platelet clumping. Stacked on top of Effient, that could nudge your bleeding risk a little higher, showing up as easier bruising, nosebleeds, or bleeding gums that takes longer to stop.

I'll be honest, this is theoretical and based only on test-tube work, not on people. Eating breadfruit as a food is very unlikely to matter. Concentrated leaf teas or extracts are the real question mark, so please run those by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Effient also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.
Severity Minor Likelihood Possible Evidence D

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the key thing about Effient (prasugrel): it's a prodrug, meaning it arrives in your body inactive and your enzymes have to switch it on before it can stop platelets from clumping. CYP3A4 is one of the enzymes that helps do that activating step. So if breadfruit gently slows CYP3A4, the theoretical concern would be slightly less active drug, which could mean a bit weaker clot protection.

Now, take a breath, because the real-world risk looks small. This worry comes only from lab dish and animal studies, not from people, and no one has shown it actually matters in patients. Eating breadfruit as a food is very unlikely to be an issue. Concentrated leaf teas or extracts are the bigger unknown. Since Effient is protecting your heart, please check with your pharmacist or doctor before using breadfruit products medicinally.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

From our pharmacist’s overview on the Effient page

“Effient (prasugrel) keeps platelets from forming clots, usually after a stent or a heart attack, and I would not pair supplements with it casually. Roughly 200 ingredients on this list can thin the blood further and raise bleeding risk, including two of the five rated major.”

About Breadfruit

What is Breadfruit?

  • Source of nutrition and energy
  • High blood pressure
  • Diabetes and blood sugar support
  • Inflammation and joint discomfort
  • Skin conditions

Breadfruit is a starchy tropical fruit eaten as a staple food, and its leaves are used in traditional medicine for conditions like high blood pressure and diabetes. As a food it is nutritious and generally safe, but the medicinal use of leaf teas or extracts has very little human research to back it up. Talk with your pharmacist or doctor before using breadfruit products for a health condition.

About Prasugrel

What Is Prasugrel Used For?

Brand name: Prasugrel

Prasugrel is used to reduce blood clot–related heart events, including clots forming in a stent (stent thrombosis). It is for people with acute coronary syndrome who are going to be treated with percutaneous coronary intervention (PCI, a procedure to open blocked heart arteries).

This includes people with unstable angina or a non-ST-elevation heart attack (NSTEMI), and people with an ST-elevation heart attack (STEMI) treated with primary or delayed PCI. Both Effient and generic Prasugrel tablets carry this use.
