Brussels Sprout and Cabotegravir

Here is the interaction we found between these two, with the evidence behind it.

Moderate

GLUCURONIDATED DRUGS

Why this applies to Cabotegravir

Cabotegravir falls under GLUCURONIDATED DRUGS — the group Brussels Sprout interacts with.

A cabbage and Brussels sprout-containing diet seems to boost elimination through glucuronide conjugation. Theoretically, these foods might also lower levels of other drugs that are metabolized through glucuronide conjugation, including acetaminophen (Tylenol, others) and oxazepam (Serax), haloperidol (Haldol), lamotrigine (Lamictal), morphine (MS Contin, Roxanol), zidovudine (AZT, Retrovir), and others.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the connection worth knowing: cabotegravir (Apretude) is cleared from your body mainly through a process called glucuronidation, the same pathway that compounds in Brussels sprouts can speed up. In theory, eating a lot of these vegetables could nudge your body to clear cabotegravir a little faster, which might lower its levels. With an HIV medicine, lower drug levels are something we take seriously because consistent levels are what keep the virus controlled or keep prevention working.

The good news: this is mostly based on limited research, not strong human studies, and normal amounts of Brussels sprouts in your meals are very unlikely to cause a problem. Concentrated supplements are a bigger unknown. Enjoy them as food, and just check with your pharmacist or doctor before using any high-dose cruciferous supplement.

From our pharmacist’s overview on the Cabotegravir page

“Cabotegravir (Apretude) prevents HIV only while its blood level stays steady, which is why this particular list, mild as it looks, deserves a real read. Every one of the interactions works through the same pathway, glucuronidation, the process the body uses to clear the drug.”

About Brussels Sprout

What is Brussels Sprout?

  • General nutrition and fiber
  • Antioxidant support
  • Digestive health
  • Heart health support
  • Cancer-prevention interest

Brussels sprouts are a nutritious cruciferous vegetable rich in fiber, vitamin C, vitamin K, and plant compounds called glucosinolates. Eating them as part of a balanced diet is healthy and safe for most people, but supplement-style claims (like preventing cancer) are not well proven. If you take blood thinners or have thyroid concerns, talk with your pharmacist or doctor about your intake.

About Cabotegravir

What Is Cabotegravir Used For?

Brand name: Apretude

Vocabria (cabotegravir) tablets are used in combination with rilpivirine (Edurant) to treat HIV-1 infection in adults and adolescents aged 12 and older who weigh at least 35 kg and whose HIV is already virologically suppressed (virus levels undetectable or very low on their current regimen). Vocabria tablets serve as a short-term oral lead-in — taken for roughly one month — before switching to long-acting injectable therapy.

Cabotegravir tablets are also used as an oral lead-in for HIV-1 pre-exposure prophylaxis (PrEP) — helping HIV-negative adults and adolescents (weighing at least 35 kg) confirm they tolerate the drug before starting injectable PrEP. Both uses require confirmed HIV-negative status before starting.
