Why this applies to Fomoterol

Fomoterol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP2C19 substrates.
In vitro evidence suggests that bacopa extract can moderately and non-competitively inhibit CYP2C19 enzymes. It is not known whether this is clinically significant.

Here's the honest picture: the concern on file is that bacopa might slow down a liver enzyme called CYP2C19, which could let certain drugs build up. With formoterol (Foradil), though, I wouldn't lose sleep over this one. Formoterol is mostly cleared through different pathways (it's broken down by other liver enzymes and largely processed by a process called glucuronidation), so CYP2C19 isn't its main exit route. It's also an inhaled medicine, so very little reaches the bloodstream to begin with.

On top of that, this warning comes only from test-tube studies, and we don't know if it matters in real people. If you ever notice extra jitteriness, a racing or pounding heart, or shakiness, mention it to your pharmacist or doctor, but a true problem here is unlikely.

Research on the effect of bacopa extracts on CYP2C9 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and non-competitively inhibit CYP2C9, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the short version: the concern on file is that bacopa might slow down a liver enzyme called CYP2C9, which could let certain drugs build up. The catch is that formoterol isn't really cleared through CYP2C9 in any meaningful way. It's mostly broken down by a different process (called glucuronidation) and other enzymes. So this particular pairing is a bit of a mismatch, and the worry doesn't translate strongly to your inhaler.

On top of that, the evidence here is very weak, coming only from lab dish studies that actually disagree with each other. In real life I'd expect little to no problem. Still, if you ever notice extra jitteriness, a racing heartbeat, or shakiness, mention it to your pharmacist or doctor.

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. What Is Formoterol Oral Inhalation Used For?

Brand name: Foradil

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
