Nortriptyline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP1A2 substrates.
Research on the effects of bacopa extracts on CYP1A2 enzymes is conflicting. Some in vitro evidence shows that bacopa extract can moderately and non-competitively inhibit CYP1A2, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the basic idea with bacopa and nortriptyline. Your body uses a liver enzyme called CYP1A2 to help clear nortriptyline. In some lab studies, bacopa appeared to slow that enzyme down a bit. If that happened in real life, nortriptyline could build up slightly, which might mean more of its usual side effects like dry mouth, constipation, drowsiness, dizziness, blurred vision, or a faster heartbeat.

That said, I want to keep this in perspective. This concern comes only from test-tube studies, and even those disagree with each other. There's no solid human evidence that bacopa meaningfully changes nortriptyline levels. I wouldn't lose sleep over it, but since tricyclics can be touchy, it's worth a quick chat with your pharmacist or doctor before adding bacopa.

Bacopa seems to inhibit acetylcholinesterase and might increase acetylcholine levels, which could counteract the effects of anticholinergic drugs. Similarly, anticholinergic drugs might counteract the cholinergic effects of bacopa.

Here's the gist: nortriptyline (Pamelor) has some anticholinergic activity, meaning part of how it works involves blocking the chemical messenger acetylcholine. Bacopa appears to do the opposite in lab studies, nudging acetylcholine levels up. So in theory, the two could push against each other and slightly blunt the effects of either one.

I want to be honest about how soft this evidence is. It's theoretical, based on how these substances behave in the lab and in animals, not on real people taking them together. I wouldn't expect a dramatic problem, but if you notice your mood treatment feeling less effective, or any new restlessness or stomach upset, that's worth mentioning. Before adding bacopa, just run it by your pharmacist or doctor so you're covered.
