Nortriptyline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Black Hoof Mushroom interacts with.

Theoretically, black hoof mushroom might increase the levels and clinical effects of CYP1A2 substrates.
In vitro, polysaccharides from black hoof mushroom grown in culture inhibit CYP1A2. This effect has not been reported in humans.

Here's the gist: your body uses a liver enzyme called CYP1A2 to help clear nortriptyline. In test tube studies, compounds from Black Hoof Mushroom appear to slow that enzyme down. If that happened in a real person, nortriptyline could build up a bit, which might mean more of its usual side effects, things like drowsiness, dry mouth, constipation, blurry vision, or a faster heartbeat.

I want to be honest about how thin this evidence is. So far this has only been seen in lab and animal studies, never in actual people, so it's mostly theoretical. Still, nortriptyline is a drug where levels matter, so before you pair the two, run it by your pharmacist or doctor so they can keep an eye on how you feel.
