These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Bacopa interacts with whole categories of medication. Acetaminophen, Dextromethorphan, Pseudoephedrine falls into 2 of those categories, so it can interact with Bacopa in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 2Why this applies to Acetaminophen, Dextromethorphan, Pseudoephedrine
Acetaminophen, Dextromethorphan, Pseudoephedrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Bacopa interacts with.
Research on the effects of bacopa extracts on CYP1A2 enzymes is conflicting. Some in vitro evidence shows that bacopa extract can moderately and non-competitively inhibit CYP1A2, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
If you're reaching for one of these combo cold and flu products, here's the honest picture. The concern on file is that bacopa might slow down a liver enzyme called CYP1A2. In theory, that could let certain medicines build up a bit. But this is based only on lab and test-tube studies, and even those results disagree with each other, so it's far from proven in real people.
None of the three active ingredients here lean heavily on CYP1A2, so a meaningful problem is unlikely. If anything did shift, you might notice slightly more of the usual effects: drowsiness or dizziness from the dextromethorphan, or jitteriness from the pseudoephedrine. None of these are prodrugs that need activating. To be safe, just run it by your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Acetaminophen, Dextromethorphan, Pseudoephedrine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo cold-and-flu product. In lab dishes, bacopa might slow down a liver enzyme called CYP3A4, which helps break down the dextromethorphan (the cough suppressant) in this medicine. If that enzyme is blocked, dextromethorphan could build up a bit, which in theory might mean more drowsiness, dizziness, or stomach upset.
The honest truth, though, is that this is very weak evidence. It comes only from test-tube studies, and even those disagree with each other. None of it has been shown to matter in real people. The acetaminophen and pseudoephedrine parts aren't really the concern here. If you take bacopa regularly, just mention it to your pharmacist so they can double-check for you.

“Ephedra is the ingredient to notice first here. Pseudoephedrine, the decongestant in Tylenol Flu, Alka-Seltzer PLUS Flu, and other acetaminophen, dextromethorphan, and pseudoephedrine combinations, is itself a stimulant, and 44 entries push the same direction, raising the odds of high blood pressure and a racing heart.”
