Erythromycin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP3A4 substrates.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the gist for combining bacopa with erythromycin. Your body uses a liver enzyme called CYP3A4 to help break erythromycin down. In the test tube, some bacopa extracts seem to slow that enzyme a bit, which could in theory let more erythromycin hang around in your system. If that happened, you might notice more of its usual side effects, like nausea, stomach cramping, or diarrhea.

I want to be honest about how shaky this evidence is. It's based only on lab studies, and even those disagree, so a real-world problem is far from certain. Most people taking a normal antibiotic course alongside bacopa won't run into trouble. Still, since erythromycin can affect heart rhythm in rare cases, it's worth a quick chat with your pharmacist or doctor before mixing the two.

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. About Erythromycin

What Is Erythromycin Used For?

Brand names: Erythro S, PCE, E-Mycin, Erythro-ES, EryPed Chewable, Eryc, Ery-Tab, Rythro Enca, Erythro E-C, Erythro Base, Ilosone, Ilotycin

Erythromycin treats bacterial infections caused by susceptible organisms. Oral and intravenous forms are used for upper and lower respiratory tract infections, skin and soft-tissue infections, pertussis (whooping cough), Legionnaires' disease, sexually transmitted infections (gonorrhea, syphilis, chlamydia), listeriosis, and other conditions. Erythrocin Lactobionate IV and erythromycin lactobionate injection are specifically for patients who cannot take oral medication or who need rapid, high blood levels due to severe infection — IV therapy should be switched to oral as soon as appropriate.

Erythromycin topical solution 2% and Ery 2% Pads (swabs) are indicated only for the topical treatment of acne vulgaris (acne) and are applied directly to the skin — they are not used for internal infections.
