Bacopa and Procainamide

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTICHOLINERGIC DRUGS

Why this applies to Procainamide

Procainamide falls under ANTICHOLINERGIC DRUGS — the group Bacopa interacts with.

Theoretically, concurrent use might decrease the effectiveness of both agents.
Bacopa seems to inhibit acetylcholinesterase and might increase acetylcholine levels, which could counteract the effects of anticholinergic drugs. Similarly, anticholinergic drugs might counteract the cholinergic effects of bacopa.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the basic idea with bacopa and procainamide. Procainamide isn't a prodrug, so it works as-is to calm down abnormal heart rhythms. Part of how it does that involves mild anticholinergic activity (it blocks some of the nerve signaling that uses acetylcholine). Bacopa, on the other hand, may gently nudge acetylcholine levels up. So in theory, the two could work against each other a little, meaning bacopa might slightly blunt how well procainamide steadies your heart rhythm.

I want to be honest though: this is a theoretical concern based on how these compounds behave in the lab, not something proven in people taking both. Still, with a heart rhythm medication, I wouldn't experiment on my own. Please check with your pharmacist or cardiologist before adding bacopa.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Procainamide page

“Procainamide treats serious heart rhythm problems, and anything that nudges the heart's electrical timing further can turn a helpful drug into a hazard, so I would give this list genuine care. Nine of the 59 supplements on file carry a major rating, and the 13 QT prolongers are the ones to understand first.”

About Bacopa

What is Bacopa?

Bacopa
  • Memory and cognitive support
  • Anxiety and stress
  • ADHD symptoms
  • Age-related memory decline
  • General brain health

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. It is generally well tolerated but can cause stomach upset, and it should be avoided in pregnancy and breastfeeding due to a lack of safety data.

About Procainamide

What Is Procainamide Used For?

Brand names: Pronestyl, Procanbid, Procan SR, Procan, Pronestyl-SR

Procainamide hydrochloride injection treats documented ventricular arrhythmias (abnormal heart rhythms starting in the lower heart chambers) that your doctor judges to be life-threatening. One example is sustained ventricular tachycardia, a fast, dangerous heartbeat.

It is generally not recommended for milder rhythm problems. It should be avoided for asymptomatic ventricular premature contractions (extra beats that cause no symptoms). The same use is listed for the Procainamide Hydrochloride, Procainamide HCl and Procainamide Hci injection products.
