Verapamil falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP1A2 substrates.
Research on the effects of bacopa extracts on CYP1A2 enzymes is conflicting. Some in vitro evidence shows that bacopa extract can moderately and non-competitively inhibit CYP1A2, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the gist for verapamil and bacopa. In lab dishes, bacopa has shown some ability to slow down a liver enzyme called CYP1A2, which is one of the pathways that helps your body clear certain medicines. The thinking is that if that enzyme is blocked, a drug it handles could build up a bit and cause stronger effects.

For verapamil specifically, that could in theory mean more of the usual effects, like lower blood pressure, dizziness, a slower heart rate, tiredness, or constipation. That said, this is theoretical and based only on test-tube studies, not real people, and even those lab results disagree with each other. Verapamil is also handled mainly by a different enzyme (CYP3A4), so I wouldn't expect a big issue. Still, since verapamil affects your heart rhythm, mention bacopa to your pharmacist or doctor before combining them.

Here's the gist for verapamil. Your body clears verapamil mainly through a liver enzyme called CYP3A4. In lab dishes, bacopa has shown it might slow that enzyme down a bit. If it did that in real life, verapamil could build up a little, which in theory could mean more of its usual effects: lower blood pressure, dizziness, a slower heartbeat, tiredness, or constipation.

The important part is that this is only a theoretical concern from test-tube and animal research, and even those studies disagree with each other. We don't have human evidence that it actually causes a problem. Still, since verapamil affects your heart rate and blood pressure, it's worth a quick mention to your pharmacist or doctor before adding bacopa, especially if you notice new lightheadedness.

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. It is generally well tolerated but can cause stomach upset, and it should be avoided in pregnancy and breastfeeding due to a lack of safety data.

What Is Verapamil Used For?

Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil

Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.

Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
