Bacopa and Acetaminophen, Dextromethorphan, Guaifenesin, Phenylpropanolamine

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Bacopa interacts with whole categories of medication. Acetaminophen, Dextromethorphan, Guaifenesin, Phenylpropanolamine falls into 2 of those categories, so it can interact with Bacopa in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 2

Why this applies to Acetaminophen, Dextromethorphan, Guaifenesin, Phenylpropanolamine

Acetaminophen, Dextromethorphan, Guaifenesin, Phenylpropanolamine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP1A2 substrates.
Research on the effects of bacopa extracts on CYP1A2 enzymes is conflicting. Some in vitro evidence shows that bacopa extract can moderately and non-competitively inhibit CYP1A2, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combination cold and cough product (Anatuss) contains acetaminophen, dextromethorphan, guaifenesin, and phenylpropanolamine. The concern on file is that bacopa might slow down a liver enzyme called CYP1A2, which in theory could let some medicines linger a bit longer in your system. If that happened, you might notice slightly stronger effects from the cough and cold ingredients, like drowsiness or dizziness from the dextromethorphan.

Here's the honest part: this interaction is theoretical and comes only from lab studies, and even those results disagree. Some show mild blocking, others say it likely doesn't matter in people. So I wouldn't lose sleep over it, especially with short-term cold relief. Still, if you take bacopa regularly, just mention it to your pharmacist so they can double-check for you.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Acetaminophen, Dextromethorphan, Guaifenesin, Phenylpropanolamine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, bacopa might increase the levels and adverse effects of CYP3A4 substrates.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

This combination cold-and-cough product (Anatuss) has one ingredient worth keeping an eye on here: dextromethorphan, the cough suppressant. Bacopa might slow down a liver enzyme called CYP3A4, which is one of the pathways that helps clear dextromethorphan. In theory, that could let a little more of it build up, nudging you toward side effects like drowsiness, dizziness, or feeling a bit foggy.

I want to be honest about how thin this evidence is. It comes only from lab dish studies, and even those disagree about whether bacopa affects this enzyme enough to matter in real people. For most folks taking a short course of a cold remedy, this is unlikely to cause trouble. Still, if you take bacopa regularly, mention it to your pharmacist or doctor.

Bacopa
  • Memory and cognitive support
  • Anxiety and stress

Bacopa is an Ayurvedic herb most often used for memory and thinking. It is generally well tolerated but can cause stomach upset, and it should be avoided in pregnancy and breastfeeding due to a lack of safety data.
