Theoretically, bacopa might increase the levels and adverse effects of CYP2C19 substrates.
In vitro evidence suggests that bacopa extract can moderately and non-competitively inhibit CYP2C19 enzymes. It is not known whether this is clinically significant.

Here's the situation with bacopa and fosphenytoin (Sesquient). Once it's in your body, fosphenytoin turns into phenytoin, and one of the liver enzymes that helps clear phenytoin is called CYP2C19. In test-tube studies, bacopa seems to slow that enzyme down a bit. In theory, that could let phenytoin levels creep higher, which might mean more side effects like drowsiness, dizziness, unsteady walking, slurred speech, or shaky eye movements.

I want to be honest, though: this is based only on lab work, not on real patients, so we truly don't know if it matters in everyday life. Because phenytoin is a fussy drug where small level changes can count, please don't add bacopa without first checking with your doctor or pharmacist.

Research on the effect of bacopa extracts on CYP2C9 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and non-competitively inhibit CYP2C9, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the thing to know about fosphenytoin: once it's in your body it converts to phenytoin, which your liver clears mainly through an enzyme called CYP2C9. Phenytoin is a narrow-therapeutic-index drug, meaning the gap between a helpful level and a too-high level is small. In lab studies, bacopa might slow down CYP2C9, and if that happened in people, phenytoin could build up. That could mean more side effects like dizziness, unsteady walking, slurred speech, drowsiness, or eye twitching.

I want to be honest, though: this is theoretical and based only on test-tube data, and even those results conflict. It may not matter at all in real life. Still, because phenytoin is so dose-sensitive, please run this by your pharmacist or doctor before adding bacopa.

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies.
