Galantamine Extended Release falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bacopa interacts with.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for galantamine ER (Razadyne ER). Your liver uses an enzyme called CYP3A4 to help clear this drug from your body. In lab studies, bacopa may mildly block that enzyme. If it does, a little more galantamine could linger, which in theory could nudge up its usual side effects like nausea, vomiting, diarrhea, dizziness, a slow heartbeat, or stomach cramps.
I want to be honest, though: this is only test-tube evidence, and even those studies disagree. We don't have real-world cases showing a problem, so it's more of a theoretical caution than a sure thing. Since galantamine can already be tough on the stomach and heart rate, it's worth a quick chat with your pharmacist or doctor before adding bacopa.

Bacopa seems to inhibit acetylcholinesterase and might increase acetylcholine levels. Theoretically, this could result in additive cholinergic effects when used with cholinergic drugs.

Here's the gist: Galantamine ER (Razadyne ER) works by blocking an enzyme called acetylcholinesterase, which raises levels of acetylcholine, a brain messenger tied to memory. Bacopa appears to nudge things in the same direction in lab studies. So when you take them together, the worry is a bit of a 'doubling up' effect, where the two could add to each other.
If that happens, you might notice more of galantamine's usual side effects, like nausea, stomach upset, diarrhea, dizziness, or a slower heart rate. I want to be honest, though: this concern is theoretical, based on how each one works rather than real cases in people. Bacopa is an Ayurvedic herb most often used for memory and thinking.
