Nortriptyline falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cajeput Oil interacts with.

Evidence from in vitro research suggests that cajeput leaf extract can inhibit cytochrome P450 2D6 (CYP2D6) by approximately 30%. Theoretically, cajeput oil may increase levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the gist for nortriptyline. Your body relies on a liver enzyme called CYP2D6 to break this antidepressant down and clear it out. Lab studies hint that cajeput leaf extract can slow that enzyme by roughly 30 percent. If that happens in real life, nortriptyline could linger longer and build up, which may turn up its effects and side effects, things like drowsiness, dry mouth, constipation, blurry vision, dizziness on standing, or a faster heartbeat.

I want to keep this in perspective though. The evidence is only test-tube data, not human studies, so this is more theoretical than proven. Still, nortriptyline has a fairly narrow safe range, so even small shifts can matter. Before using cajeput oil, check with your pharmacist or doctor first.
