Fentanyl falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cajeput Oil interacts with.

Evidence from in vitro research suggests that cajeput leaf extract can inhibit cytochrome P450 2D6 (CYP2D6) by approximately 30%. Theoretically, cajeput oil may increase levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the gist for fentanyl. The concern on file is that compounds in cajeput oil might block a liver enzyme called CYP2D6, and fentanyl is partly handled by that enzyme. Fentanyl is an active drug (not a prodrug), so if its breakdown slows down, more of it could linger in your body. In theory that means stronger opioid effects, like heavy drowsiness, confusion, slowed or shallow breathing, and dangerously low blood pressure.

The important catch: this comes from lab studies only, not from people, and CYP3A4 actually does most of fentanyl's breakdown, not CYP2D6. So the real-world risk is likely small. Still, fentanyl is a powerful opioid with little room for error, so please run any cajeput oil use past your doctor or pharmacist first.

What Is Fentanyl Used For?

Fentanyl transdermal system is prescribed for severe, persistent pain that cannot be controlled with other pain medicines, including shorter-acting opioids. It is only for people who are already opioid-tolerant — meaning they've been taking a significant daily opioid dose for at least a week.
