Tramadol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cajeput Oil interacts with.

Evidence from in vitro research suggests that cajeput leaf extract can inhibit cytochrome P450 2D6 (CYP2D6) by approximately 30%. Theoretically, cajeput oil may increase levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the wrinkle with tramadol: it's actually a bit of a two-sided drug. Your liver uses an enzyme called CYP2D6 to turn part of tramadol into a stronger pain-relieving form (called O-desmethyltramadol). Lab studies hint that compounds in cajeput may slightly block that enzyme, by roughly 30 percent in a test tube.

If that happened in the body, it could mean a bit less of tramadol's main pain relief, since less is converted to the active form. At the same time, the parent drug can hang around, which still carries side effects like drowsiness, nausea, or constipation. Honestly though, this is only test-tube data, so the real-world effect is uncertain. Swallowing cajeput oil is never safe anyway. If you use it, just mention it to your pharmacist.
