Bacopa and Tramadol, Acetaminophen

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Bacopa interacts with whole categories of medication. Tramadol, Acetaminophen falls into 2 of those categories, so it can interact with Bacopa in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 2

Why this applies to Tramadol, Acetaminophen

Tramadol, Acetaminophen falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP1A2 substrates.
Research on the effects of bacopa extracts on CYP1A2 enzymes is conflicting. Some in vitro evidence shows that bacopa extract can moderately and non-competitively inhibit CYP1A2, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist with Tramacet (tramadol plus acetaminophen) and bacopa. In the lab, bacopa may mildly slow down a liver enzyme called CYP1A2. In theory, if that enzyme is slowed, a drug it normally clears could build up a bit and cause stronger effects. With tramadol, that might mean more drowsiness, dizziness, nausea, or constipation than usual.

That said, I want to be honest about how thin this evidence is. It comes only from test-tube studies, and even those disagree, so a real-world problem is far from certain. Tramadol also leans more heavily on other enzymes than CYP1A2. If you take these together regularly, just give your pharmacist or doctor a heads-up and watch for extra sleepiness.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Tramadol, Acetaminophen also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, bacopa might increase the levels and adverse effects of CYP3A4 substrates.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the gist with Tramacet (tramadol plus acetaminophen) and bacopa. In lab dish studies, bacopa might slow down a liver enzyme called CYP3A4, which is one of the routes your body uses to process the tramadol part. If that enzyme runs slower, a bit more tramadol could hang around, which in theory might bump up its effects like drowsiness, dizziness, nausea, or constipation.

That said, I want to be honest: this is only a theoretical concern from test-tube research, and even those studies disagree. It's not something proven to cause problems in real people. Still, if you notice extra grogginess after adding bacopa, mention it to your pharmacist or doctor before changing anything.

From our pharmacist’s overview on the Tramadol, Acetaminophen page

“The combinations I worry about most with tramadol and acetaminophen (Tramacet) involve anything that deepens drowsiness or strains the liver. Tramadol is an opioid, so sedating supplements like kava, along with alcohol in any form, can slow breathing on top of adding sleepiness, which is why this list deserves genuine care rather than…”

About Bacopa

What is Bacopa?

Bacopa
  • Memory and cognitive support
  • Anxiety and stress
  • ADHD symptoms
  • Age-related memory decline
  • General brain health

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. It is generally well tolerated but can cause stomach upset, and it should be avoided in pregnancy and breastfeeding due to a lack of safety data.
