Telmisartan, Amlodipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP3A4 substrates.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Twynsta combines two blood pressure medicines. The amlodipine half is handled in your body partly by a liver enzyme called CYP3A4. In test-tube studies, bacopa may slightly slow that enzyme down. If that happened in real life, amlodipine could build up a little, which might lead to more of its usual effects like swollen ankles, lightheadedness, flushing, or your blood pressure dropping lower than expected.

Here's my honest take though: this is only a theoretical concern from lab data, and even those studies disagree on whether it matters. Neither of these drugs is a prodrug, so the direction would simply be a touch stronger, not weaker. For most folks this isn't a big deal, but if you notice new dizziness or swelling, let your pharmacist or doctor know.

Bacopa is an Ayurvedic herb most often used for memory and thinking.
