Most of the 235 ingredients on file fall into two patterns. Many can nudge blood pressure further down than the drug intends, or push back against it, and a large group affects the CYP3A4 enzyme that clears amlodipine. Grapefruit blocks that enzyme and can raise drug levels; St. John's wort revs it up and can quietly weaken the pill.
The remaining risks are quieter but worth knowing. Because telmisartan makes the kidneys hold on to potassium, high-dose potassium supplements can push blood levels too high.
