Telmisartan, Amlodipine falls under ANTIHYPERTENSIVE DRUGS — the group Danshen interacts with.

Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Twynsta combines two blood pressure medicines: telmisartan, which relaxes your vessels by blocking a hormone called angiotensin, and amlodipine, a calcium channel blocker that also widens your arteries. Danshen seems to lower blood pressure on its own, so stacking it on top of these two could push your pressure down a little too far.

If that happens, you might feel lightheaded, dizzy, tired, or faint, especially when you stand up quickly. I want to be honest though: this concern comes mainly from animal and lab studies, not solid human trials, so it's a theoretical worry rather than a proven one. Danshen can also thin the blood, which is a separate caution. Before adding it, please check with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Telmisartan, Amlodipine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

In your Twynsta tablet, the amlodipine half is the piece that matters here. It's cleared by a liver and gut enzyme called CYP3A4, and Danshen can nudge that enzyme in either direction. When taken for a couple of weeks, Danshen seems to speed up that enzyme, which could lower amlodipine levels and let your blood pressure creep back up. A single recent dose may do the opposite and push amlodipine higher, which could mean dizziness, flushing, swollen ankles, or a faster heartbeat.

This is based on early human studies using a different test drug, so it's a possible concern rather than a sure thing. The telmisartan side isn't really affected. Please don't change anything on your own. Just check with your pharmacist or doctor before adding Danshen.

Theoretically, taking danshen in combination with amlodipine may decrease the clinical effects of amlodipine.
In animal research, taking danshen orally in combination with amlodipine reduced blood levels of amlodipine by about 52%. This may have been due to induction of cytochrome P450 3A4 (CYP3A4) by danshen, which has been demonstrated in vitro. So far, this interaction has not been reported in humans.

Twynsta combines two blood pressure medicines: telmisartan and amlodipine. The interaction on file is with the amlodipine half. In animal studies, Danshen seemed to speed up an enzyme called CYP3A4, which is one of the main ways your body clears amlodipine. When that enzyme runs faster, amlodipine can get cleared out sooner, so less of it stays in your blood (one animal study saw levels drop by about half).

In plain terms, Danshen could make your amlodipine work less well, so your blood pressure might not be as controlled. The honest caveat: this is based on lab and animal research and hasn't actually been reported in people. Still, since Danshen can also affect blood clotting, please check with your pharmacist or doctor before combining them rather than changing anything on your own.
