Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Twynsta already does a solid job lowering your blood pressure since it pairs two medicines: telmisartan relaxes your blood vessels, and amlodipine widens them too. Flaxseed oil isn't broken down by the same pathways, so this isn't really a chemistry clash. The thought here is simpler: some research hints that taking flaxseed oil over the long term may modestly nudge blood pressure down on its own. Stack that on top of Twynsta and, in theory, the two could add up.

If your numbers dropped too far, you might feel dizzy, lightheaded, or a bit faint, especially standing up quickly. That said, the evidence is mixed and mostly theoretical, so for many people it's a non-issue. Just keep an eye on your readings and check with your pharmacist or doctor before adding it.
