Telmisartan, Amlodipine falls under ANTIHYPERTENSIVE DRUGS — the group Dendrobium interacts with.

The dendrobium constituent dendrobine is reported to have hypotensive effects. Theoretically, combining dendrobium with antihypertensive drugs might increase the risk of hypotension; use with caution.

Twynsta combines two blood pressure medicines: telmisartan (which relaxes blood vessels) and amlodipine (a calcium channel blocker that does the same in a different way). Here's the thing to know: one of the compounds in dendrobium, called dendrobine, has been reported to lower blood pressure on its own. So if you pair it with Twynsta, the two could add up and push your pressure down more than expected.

I'll be honest, this concern is theoretical and based on how dendrobine behaves, not on real-world reports in people, so for most folks the risk is low. Still, watch for dizziness, lightheadedness, feeling faint when you stand up, or unusual tiredness. Don't change anything on your own. Just give your pharmacist or doctor a heads-up before adding dendrobium.
