Dendrobium and Cetirizine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

SEIZURE THRESHOLD LOWERING DRUGS

Why this applies to Cetirizine

Cetirizine falls under SEIZURE THRESHOLD LOWERING DRUGS — the group Dendrobium interacts with.

The dendrobium constituent dendrobine is reported to have convulsant effects. Theoretically, combining dendrobium with pro-convulsant drugs might increase the risk of seizure. Some drugs that lower the seizure threshold include anesthetics (propofol, others), antiarrhythmics (mexiletine), antibiotics (amphotericin, penicillin, cephalosporins, imipenem), antidepressants (bupropion, others), antihistamines (cyproheptadine, others), immunosuppressants (cyclosporine), narcotics (fentanyl, others), stimulants (methylphenidate), theophylline, and others.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist of it. One of Dendrobium's natural compounds, called dendrobine, has been reported in lab settings to have convulsant effects, meaning it could in theory make seizures a bit more likely. Cetirizine (Zyrtec) is an antihistamine, and antihistamines as a group can slightly lower the seizure threshold too. So the concern is that stacking the two together might add up.

I want to be honest about how soft this evidence is. This is purely theoretical, based on how these compounds behave in the lab, not on real patient reports. For most healthy people taking ordinary Cetirizine doses, this isn't likely to be a problem. But if you have epilepsy or a seizure history, please run it past your pharmacist or doctor first before combining them.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Cetirizine page

“Cetirizine (Zyrtec) treats allergies, but even this newer antihistamine can make some people drowsy, and that weak spot is where most of its 100 documented supplement interactions land. About Dendrobium

What is Dendrobium?

  • Dry mouth and thirst
  • Supporting digestion
  • General tonic/vitality
  • Eye and skin health
  • Recovery after illness

Dendrobium is a group of orchids long used in traditional Chinese medicine, often to moisten the body and as a general tonic. Human evidence for its modern health claims is very limited, so it should be viewed as a traditional remedy rather than a proven treatment. Talk to your pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding because safety isn't known.
