Dendrobium and Amphetamine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

SEIZURE THRESHOLD LOWERING DRUGS

Why this applies to Amphetamine

Amphetamine falls under SEIZURE THRESHOLD LOWERING DRUGS — the group Dendrobium interacts with.

The dendrobium constituent dendrobine is reported to have convulsant effects. Theoretically, combining dendrobium with pro-convulsant drugs might increase the risk of seizure. Some drugs that lower the seizure threshold include anesthetics (propofol, others), antiarrhythmics (mexiletine), antibiotics (amphotericin, penicillin, cephalosporins, imipenem), antidepressants (bupropion, others), antihistamines (cyproheptadine, others), immunosuppressants (cyclosporine), narcotics (fentanyl, others), stimulants (methylphenidate), theophylline, and others.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for amphetamine (like Dyanavel XR, Mydayis, or Adzenys): a compound in Dendrobium called dendrobine has been reported to act as a convulsant, meaning it may make seizures a little easier to trigger. Amphetamine itself is a stimulant that can also nudge your seizure threshold lower. Put the two together and, in theory, you could raise the odds of a seizure, especially if you're already prone to them.

I want to be straight with you though: this concern is theoretical, based on how these substances behave in the lab and in animals, not on real reports in people. For most folks it likely won't be a problem, but if you have a history of seizures or feel jittery, please check with your pharmacist or doctor before combining them.

Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.

About Dendrobium

What is Dendrobium?

  • Dry mouth and thirst
  • Supporting digestion
  • General tonic/vitality
  • Eye and skin health
  • Recovery after illness

Dendrobium is a group of orchids long used in traditional Chinese medicine, often to moisten the body and as a general tonic. Human evidence for its modern health claims is very limited, so it should be viewed as a traditional remedy rather than a proven treatment. Talk to your pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding because safety isn't known.
