Telmisartan, Amlodipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Syrian Rue interacts with.

Theoretically, Syrian rue might increase the levels and clinical effects of drugs metabolized by CYP3A4.
Syrian rue contains beta-carboline alkaloids, including harmine, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids competitively inhibit CYP3A4 enzymes.

In Twynsta, the amlodipine part is the piece to watch here. Your body relies on an enzyme called CYP3A4 to break amlodipine down. Lab research hints that the harmala alkaloids in Syrian Rue may slow that enzyme, so amlodipine could clear more slowly and build up a bit. That could mean a stronger blood-pressure-lowering effect than expected, along with side effects like dizziness, lightheadedness, headache, flushing, or swelling in your ankles. The telmisartan side isn't cleared this way, so it's mostly the amlodipine I'd keep an eye on.

Keep in mind this is theoretical, based only on test-tube data, so the real-world risk may be small. Still, given Syrian Rue's own toxicity concerns, please check with your pharmacist or doctor before combining them.
