Telmisartan, Amlodipine falls under ANTIHYPERTENSIVE DRUGS — the group Rhodiola interacts with.
In vitro and animal research shows that rhodiola extract inhibits angiotensin-converting enzyme (ACE) and might lower blood pressure.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Twynsta combines two blood pressure medicines: telmisartan, which blocks a hormone that tightens your blood vessels, and amlodipine, which relaxes those vessels. Rhodiola may have a mild blood-pressure-lowering effect of its own. The concern here is simple addition: if the herb nudges your pressure down while your medication is already doing that job, the two together could lower it a bit too much.
I want to be honest about how thin this evidence is. It comes only from lab and animal studies, not from people, so this is more of a theoretical caution than a proven problem. Still, if you take Rhodiola with Twynsta, watch for dizziness, lightheadedness, or feeling faint when you stand. Please check with your pharmacist or doctor before combining them, and don't change your medication on your own.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Telmisartan, Amlodipine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Twynsta combines two blood pressure medicines, and the amlodipine part is partly cleared by a liver enzyme called CYP3A4. In a test tube, rhodiola can slow that enzyme down, which in theory could let amlodipine build up a little. If that happened, you might notice more of amlodipine's usual effects, like dizziness, flushing, headache, or swelling in your ankles. Telmisartan isn't really affected by this enzyme.
Here's the reassuring part: this is mostly a lab finding, and the effect varies a lot between products. When researchers tested rhodiola in real people using another CYP3A4 drug, they saw no meaningful change. So a real-world problem is unlikely, but if you're considering rhodiola, it's worth a quick word with your pharmacist or doctor first.
