Twynsta combines two blood pressure medicines: telmisartan (an ARB) and amlodipine (a calcium channel blocker). Both work to lower your blood pressure. Here's the thing about noni: some early research hints that drinking noni juice may nudge blood pressure down on its own. So when you pair it with a medicine already doing that job, the two could add up and push your numbers lower than intended.

If that happens, you might feel dizzy, lightheaded, unusually tired, or notice it more when you stand up quickly. I'll be honest with you, this is based mostly on anecdotal reports, not strong studies, so it's not a sure thing. Still, it's worth a quick chat with your pharmacist or doctor before adding regular noni, and let them know if you feel woozy.

Theoretically, combining noni and ARBs might increase the risk of hyperkalemia.
Noni juice contains significant amounts of potassium, about 6 mEq/100 mL juice. This may increase the risk for hyperkalemia when used in conjunction with ARBs, which can also increase potassium levels.

Here's the gist with Twynsta (telmisartan plus amlodipine) and noni. The telmisartan half is an ARB, and one thing ARBs naturally do is nudge your potassium levels up a bit. Noni juice happens to carry a fair amount of potassium on its own (roughly 6 mEq per 100 mL). So in theory, pairing the two could push potassium higher than you'd want, a condition called hyperkalemia.

To be honest, this is based on weak, anecdotal evidence, so I wouldn't lose sleep over an occasional sip. Still, if potassium climbs too high, it can cause muscle weakness, tingling, or in rare cases an irregular heartbeat. If you have kidney trouble or drink noni regularly, please check with your pharmacist or doctor first.
