Here's the simple version: noni juice may nudge your blood pressure down a little on its own. Since spironolactone is already working to lower your blood pressure (and help your body shed extra fluid), pairing the two could add up and push your pressure too low. That might leave you feeling dizzy, lightheaded, or woozy, especially when you stand up quickly.

I want to be honest about how solid this is: the concern is mostly theoretical and based on weak, anecdotal reports, not strong studies. One extra thing worth mentioning is that spironolactone raises potassium, and some noni products are surprisingly high in potassium too. Please check with your pharmacist or doctor before combining them, rather than stopping anything on your own.

Theoretically, combing noni and a potassium-sparing diuretic might increase the risk of hyperkalemia.
Noni juice contains significant amounts of potassium, about 6 mEq/100 mL juice. This may increase the risk for hyperkalemia when used in conjunction with potassium-sparing diuretics, which can also increase potassium levels.

Here's the thing to keep in mind with spironolactone: it's a potassium-sparing diuretic, which means it helps your body hold onto potassium instead of flushing it out. That's usually a good thing, but it also means your potassium can climb higher than you'd want. Noni juice naturally contains a fair amount of potassium (roughly 6 mEq per 100 mL), so drinking it regularly alongside spironolactone could nudge your levels even higher.

If potassium gets too high (a state called hyperkalemia), it can cause muscle weakness, a slow or irregular heartbeat, or tingling. To be fair, this is mostly a theoretical concern based on limited reports, not strong studies. An occasional sip likely won't matter, but daily supplement-sized amounts could. Please check with your pharmacist or doctor before combining them, especially if you have kidney trouble.

Noni is a tropical fruit used widely in folk medicine and sold mostly as a juice or supplement, but solid human evidence for its many claimed benefits is limited. About Spironolactone

What Is Spironolactone Used For?

Brand names: Aldactone, Spiroton, Carospir

Spironolactone tablets (including Aldactone) are used to treat moderate-to-severe heart failure, high blood pressure, and fluid retention (edema) caused by liver cirrhosis or nephrotic syndrome. They are also used to diagnose and manage primary hyperaldosteronism — a condition where the adrenal glands produce too much of the hormone aldosterone.

Spironolactone oral suspension shares several of these uses — heart failure, high blood pressure, and edema from liver cirrhosis — but is not the same as Aldactone and is not therapeutically interchangeable with it. Your prescriber will choose the right form for your situation.
