Here's the simple version: both noni and diclofenac can each be hard on the liver, so the worry is about doubling up on that risk. Diclofenac (Voltaren) is a common anti-inflammatory used for arthritis and pain, and like other NSAIDs it occasionally raises liver enzymes. Noni juice has its own rare reports of liver trouble. This isn't about one changing how the other works, it's about adding two potential liver stressors together.

To be clear, this is a theoretical, anecdotal concern, not a proven one. Most people tolerate either just fine. Still, watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before combining them, especially if you have liver or kidney issues.

Diclofenac sodium topical gel is used to treat actinic keratoses (AK) — rough, scaly skin patches that form after years of sun exposure. These patches are considered precancerous, meaning they can develop into skin cancer if left untreated.

This is a topical gel, meaning it is applied directly to the skin over the affected area rather than taken by mouth. It belongs to the NSAID (nonsteroidal anti-inflammatory drug) family.
