Telmisartan, Amlodipine falls under ANTIHYPERTENSIVE DRUGS — the group Oregon Grape interacts with.

Theoretically, Oregon grape might increase the risk of hypotension when taken with antihypertensive drugs.
Animal research suggests that berberine, a constituent of Oregon grape, can have hypotensive effects. Also, an analysis of clinical evidence suggests that taking berberine in combination with amlodipine (Norvasc) can lower systolic and diastolic blood pressure when compared with taking amlodipine alone.

Here's the gist with Twynsta (telmisartan and amlodipine): both ingredients already lower your blood pressure, telmisartan by relaxing blood vessels and amlodipine by easing the flow through them. Oregon grape contains berberine, which in animal studies and some early human data appears to nudge blood pressure down a little on its own. So the worry is simply additive, the two together might push your numbers lower than intended.

If that happens, you could feel dizzy or lightheaded, especially standing up quickly, tired, or even faint. This concern is mostly theoretical and based on lab and animal work, so don't panic. Still, before adding an Oregon grape supplement, check with your pharmacist or doctor, and let them know if you notice any low-pressure symptoms.

Theoretically, Oregon grape might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of Oregon grape, moderately inhibits cytochrome P450 3A4 (CYP3A4).

Here's the gist with Twynsta (telmisartan plus amlodipine). The amlodipine half is cleared by a liver enzyme called CYP3A4. Berberine, found in Oregon grape root, may gently slow that enzyme down. In theory, that could let amlodipine build up a little, which might nudge your blood pressure lower than expected or bring on amlodipine's usual side effects, like ankle swelling, flushing, dizziness, or a headache.

I want to be honest though: this is based on lab work and early data, not strong human studies, so it's more of a maybe than a sure thing. Food-sized exposure isn't really the concern; concentrated supplements are. Before adding Oregon grape, check in with your pharmacist or doctor, and keep an eye on your blood pressure readings.
