Telmisartan, Amlodipine falls under CYTOCHROME P450 2C19 (CYP2C19) INHIBITORS — the group Cannabidiol (cbd) interacts with.

Theoretically, CYP2C19 inhibitors might increase cannabidiol levels.
Cannabidiol is a substrate of CYP2C19 enzymes. Theoretically, drugs that inhibit CYP2C19 enzymes might increase levels of cannabidiol, increasing its effects and adverse effects.

Here's the key thing to understand: in this pairing, it's the CBD that may be affected, not your Twynsta. One of the two ingredients in Twynsta can mildly slow down a liver enzyme called CYP2C19, which happens to be one of the enzymes your body uses to break down CBD. So in theory, taking them together could let CBD build up a little more than usual, which might mean stronger CBD effects like drowsiness, lightheadedness, or an upset stomach.

I want to be honest that this is a theoretical concern based on how the drugs work on paper, not something proven in real patients. Your blood pressure medicine itself shouldn't become more or less effective. Still, it's worth a quick chat with your pharmacist or doctor before adding CBD, especially since CBD product quality varies so much.

Cannabidiol may increase levels of drugs that are metabolized by CYP3A4.
In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the part of Twynsta that matters for CBD: the amlodipine half is cleared by an enzyme called CYP3A4, and CBD can slow that enzyme down. When the enzyme works slower, amlodipine can build up a bit more than usual. That could nudge its blood-pressure-lowering effect stronger, so you might notice things like dizziness, lightheadedness when standing up, swollen ankles, flushing, or a faster heartbeat. The telmisartan part isn't really affected this way.

This is a real, well-documented effect with CBD, though how much it matters depends on your CBD dose and product. Most people do fine, but please don't change anything on your own. Just check in with your pharmacist or doctor first, and keep an eye on your blood pressure if you start CBD.
