Here's the gist for combining CBD with your phenytoin (Dilantin). On paper, CBD can have its own mild calming or sedating effect when used at higher doses. Since phenytoin can also make some people feel drowsy or a bit foggy, putting the two together might add up, so you could feel sleepier, dizzier, or more tired than usual. I want to be honest though: this concern is mostly theoretical, based on how these compounds behave rather than strong human studies.

One extra wrinkle: phenytoin is a tricky drug where small shifts in blood levels can matter, and CBD may affect how the liver handles it. So please loop in your pharmacist or neurologist before starting CBD. Don't change anything on your own.

Research shows that cannabidiol inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabidiol with CYP2C19 substrates may increase the risk for adverse effects from these substrates.

Here's the thing with phenytoin and CBD. Your body uses a liver enzyme called CYP2C19 to help break phenytoin down, and CBD slows that enzyme down. When the cleanup process slows, phenytoin can build up in your blood. Phenytoin is what we call a narrow therapeutic index drug, meaning the gap between a helpful level and a too-high level is small, so even a modest rise matters.

If levels climb too high, you might notice dizziness, slurred speech, unsteady walking, trouble with balance, drowsiness, or twitchy eye movements. I'd never want you to stop or change either one on your own. The safe move is to check with your pharmacist or doctor, who may want to watch your phenytoin blood levels more closely.

Here's the thing to keep straight with this pair: the concern runs from phenytoin affecting CBD, not the other way around. Phenytoin tends to speed up a liver enzyme called CYP2C19, and CBD is partly cleared by that same enzyme. So in theory, taking them together could push your CBD levels down, meaning the CBD may not work as well as you'd expect.

The good news is that this is a theoretical interaction based on how the drugs behave, not on strong real-world studies, so I wouldn't lose sleep over it. Here's the gist with phenytoin and CBD. CBD has been shown in lab dish studies to block an enzyme called CYP2C8, which is one of the pathways your body uses to clear phenytoin. If that pathway slows down, phenytoin levels could theoretically creep up. With a drug like phenytoin, that matters, because it has a very narrow window between a helpful dose and too much. Higher levels can bring on things like dizziness, slurred speech, unsteady walking, double vision, drowsiness, or nausea.

That said, I want to keep this in perspective. This is only a theoretical, test-tube finding and hasn't actually been reported in people. Still, since phenytoin is so dose-sensitive, please loop in your doctor or pharmacist before combining them. A simple blood level check can keep you safe.

What Is Phenytoin Used For?

Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection

Phenytoin treats seizures. Oral products are used for generalized tonic-clonic (grand mal) seizures and for psychomotor (temporal lobe) or complex partial seizures. Most oral products are also used to prevent and treat seizures during or after neurosurgery.

Phenytoin Sodium Injection is used for generalized tonic-clonic status epilepticus (a prolonged, dangerous seizure emergency). It is also used for seizures during neurosurgery. The injection should be used only when taking phenytoin by mouth is not possible. Dilantin-125 and Phenytoin oral suspension are listed for tonic-clonic and psychomotor seizures.
