Why this applies to Propranolol

Propranolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Theoretically, cannabidiol might increase levels of drugs metabolized by CYP2D6.
In vitro research shows that cannabidiol inhibits CYP2D6. Theoretically, concomitant use of cannabidiol with CYP2D6 substrates might increase the risk for adverse effects from these substrates. However, a clinical crossover trial in healthy adults shows that cannabidiol does not inhibit dextromethorphan, a substrate of CYP2D6.

Here's the gist for propranolol and CBD. Your liver uses an enzyme called CYP2D6 to help clear propranolol from your body. In lab (test tube) studies, CBD can slow that enzyme down. If that happened in real life, more propranolol could linger in your system, which theoretically could nudge its effects up a bit, think a slower heart rate, lower blood pressure, lightheadedness, or feeling more tired than usual.

The good news is this is only a theoretical concern. Propranolol is active on its own (not a prodrug), so blocking CYP2D6 would tend to raise its levels rather than weaken it. A human study with a similar CYP2D6 drug showed no real-world effect, so the risk looks low. Still, if you start CBD, mention it to your pharmacist or doctor and keep an eye on your pulse and how you feel.

Research shows that cannabidiol inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabidiol with CYP2C19 substrates may increase the risk for adverse effects from these substrates.

Here's the gist: CBD can slow down a liver enzyme called CYP2C19, which is one of the pathways your body uses to break down propranolol. When that pathway gets blocked, propranolol can build up a bit higher than usual in your bloodstream. Since propranolol isn't a prodrug (it works as-is, no activation needed), more drug hanging around means a stronger beta-blocker effect.

In real life, that could show up as a slower heart rate, lower blood pressure, lightheadedness, unusual tiredness, or cold hands and feet. The evidence that CBD blocks this enzyme is solid, though propranolol also relies on other pathways, so the size of the bump can vary. Please don't change anything on your own. Just loop in your pharmacist or doctor so they can check your pulse and blood pressure and adjust if needed.

Cannabidiol may increase levels of drugs metabolized by CYP1A2.
In vitro research shows that cannabidiol inhibits CYP1A2. Furthermore, clinical studies show that cannabidiol may inhibit the metabolism of caffeine, a CYP1A2 substrate. Two pharmacokinetic studies in healthy adults show that taking cannabidiol dosed 640 mg once up to 750 twice daily increases the area under the curve of caffeine. However, results are mixed over whether cannabidiol impacts peak serum levels of caffeine.

Here's the simple version: your liver uses an enzyme called CYP1A2 to help break down propranolol. CBD can slow that enzyme down, so propranolol may clear out of your body a little more slowly and build up. Propranolol isn't a prodrug, so more drug hanging around means a stronger effect, not a weaker one.

In real life, that could mean your heart rate or blood pressure dips lower than usual, leaving you feeling tired, dizzy, or lightheaded when you stand up. The effect is more likely with concentrated CBD products than a tiny amount. Please don't change anything on your own. What Is Propranolol (Cardiovascular) Used For?

Brand names: Inderal LA, Inderal, Angilol, Syprol, Inderal Injection, Half-Inderal LA, Bedranol SR, Half Beta Prograne, Beta Prograne, Lopranol LA, Slo-Pro, InnoPran XL

This intravenous (IV) form of propranolol is used in hospital or clinical settings to treat dangerous heart rhythm problems. It is generally reserved for life-threatening arrhythmias (abnormal heart rhythms) or those that occur during anesthesia.

Specific conditions it treats include fast heart rates coming from the upper chambers of the heart (supraventricular tachycardia), certain arrhythmias caused by digitalis toxicity, dangerous ventricular rhythms that haven't responded to other treatments, and fast heart rates caused by excess adrenaline-like substances during surgery.
