Why this applies to Theophylline

Theophylline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs metabolized by CYP1A2.
In vitro research shows that cannabidiol inhibits CYP1A2. Furthermore, clinical studies show that cannabidiol may inhibit the metabolism of caffeine, a CYP1A2 substrate. Two pharmacokinetic studies in healthy adults show that taking cannabidiol dosed 640 mg once up to 750 twice daily increases the area under the curve of caffeine. However, results are mixed over whether cannabidiol impacts peak serum levels of caffeine.

Here's the thing with theophylline: your liver uses an enzyme called CYP1A2 to clear it from your body. CBD appears to slow that enzyme down, and we've seen this play out in good-quality studies with caffeine, which the body handles the same way. So pairing CBD with theophylline could let theophylline build up higher than expected.

Why does that matter? Theophylline is what we call a narrow therapeutic index drug, meaning the helpful dose and the toxic dose are close together. Even a small rise in levels can trigger nausea, a racing or pounding heartbeat, jitteriness, trouble sleeping, or in serious cases seizures. I wouldn't panic, but please loop in your pharmacist or doctor before combining them. They can check a blood level if needed.

Theophylline — available as extended-release capsules (Theo-24), extended-release tablets, and oral solution (Elixophyllin) — is used to treat symptoms and reversible airflow obstruction caused by chronic asthma, emphysema, and chronic bronchitis.

All supported formulations share these indications. Theophylline is intended for patients with ongoing or recurring symptoms, not for sudden (acute) breathing attacks, which need faster-acting treatments.
