Indomethacin falls under CYTOCHROME P450 2C19 (CYP2C19) INHIBITORS — the group Cannabidiol (cbd) interacts with.

Theoretically, CYP2C19 inhibitors might increase cannabidiol levels.
Cannabidiol is a substrate of CYP2C19 enzymes. Theoretically, drugs that inhibit CYP2C19 enzymes might increase levels of cannabidiol, increasing its effects and adverse effects.

Here's the key thing to understand about this pairing: it's the CBD levels that may go up, not the indomethacin. Your body uses an enzyme called CYP2C19 to help break down CBD, and indomethacin can mildly slow that enzyme down. So in theory, less CBD gets cleared and it can build up a bit, possibly making CBD's own effects (like drowsiness, diarrhea, or feeling more tired) a little stronger.

I'll be honest with you though, this is only a theoretical concern based on how the enzyme works, not something proven in real patients. The bigger, more practical worry is that both indomethacin (an NSAID) and CBD can be tough on the stomach and liver. If you use both regularly, it's worth a quick chat with your pharmacist or doctor.

Indomethacin also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Cannabidiol may increase levels of drugs metabolized by CYP2C9.
In vitro and animal research shows that cannabidiol inhibits CYP2C9. In human studies, cannabidiol has been associated with an increase in plasma levels of topiramate and losartan, CYP2C9 substrates. Concomitant use of cannabidiol with CYP2C9 substrates may increase the risk for adverse effects from these substrates.

Here's the gist: your body uses an enzyme called CYP2C9 to help clear indomethacin, and CBD can slow that enzyme down. When that happens, indomethacin can build up a little higher than usual in your system. Since indomethacin is already one of the stronger anti-inflammatories, more of it can mean more of its typical downsides, things like stomach upset, heartburn, an increased risk of stomach bleeding or ulcers, headaches, dizziness, and added strain on the kidneys.

This isn't a reason to panic, and indomethacin is not a prodrug, so the effect goes in this direction: a bit stronger, not weaker. The evidence is fairly solid for CYP2C9 in general, but the size of the change varies by person and product. Please loop in your pharmacist or doctor before combining them so they can watch for side effects.

About Indomethacin

What Is Indomethacin Used For?

Brand names: Indocin, Indocin SR, Indo-Lemmon, Indomethagan, Tivorbex

Indomethacin oral suspension and rectal suppository forms (including Indocin oral suspension and Indocin suppositories) are used to treat moderate to severe rheumatoid arthritis (including acute flares), moderate to severe ankylosing spondylitis (spine inflammation), moderate to severe osteoarthritis, acute painful shoulder (bursitis and/or tendinitis), and acute gouty arthritis. Oral capsules — both immediate-release and extended-release — share these same approved uses, except that extended-release capsules are not approved for acute gouty arthritis.

Indomethacin for Injection (intravenous) is a completely separate use: it is given to premature infants to close a hemodynamically significant patent ductus arteriosus (a blood vessel near the heart that should close after birth but hasn't) when standard medical management has not worked.
