Cannabidiol (cbd) and Metronidazole

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS

Why this applies to Metronidazole

Metronidazole falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Cannabidiol (cbd) interacts with.

Theoretically, CYP3A4 inhibitors might increase cannabidiol levels.
Cannabidiol is a substrate of CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 enzymes might increase levels of cannabidiol, increasing its effects and adverse effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with these two: metronidazole can gently slow down a liver enzyme called CYP3A4, and that same enzyme helps break down CBD. So in theory, taking them together could let CBD build up a bit more than usual, which may nudge up CBD's own effects, things like drowsiness, an upset stomach, or feeling more tired than normal.

I want to be honest about the evidence here. This is a theoretical concern based on how the two work, not something proven in real patients, so don't panic. Metronidazole courses are also usually short. Still, if you notice extra sleepiness or stomach upset, mention it to your pharmacist or doctor before changing anything.

From our pharmacist’s overview on the Metronidazole page

“Metronidazole (Flagyl) can stretch the heart's QT interval on its own, and most of the seven major-rated interactions among its 83 flagged supplement ingredients do the same thing, so I read this list with more wariness than most antibiotics earn. Grapefruit and ephedra stand out for adding to that heart-rhythm risk, while activated ch…”
