Everolimus falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Cannabidiol (cbd) interacts with.

Theoretically, CYP3A4 inhibitors might increase cannabidiol levels.
Cannabidiol is a substrate of CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 enzymes might increase levels of cannabidiol, increasing its effects and adverse effects.

Here's how this one works. Both CBD and everolimus pass through the same liver pathway, called CYP3A4. In this case, everolimus can act as a mild blocker of that pathway. When CYP3A4 is slowed down, the CBD you take can build up a bit higher than expected, which may make CBD's own effects (like drowsiness, diarrhea, or feeling tired) more noticeable. Everolimus is not a prodrug, so it doesn't need this enzyme to become active.

I'll be honest with you: this is a theoretical concern based on how these drugs are processed, not something proven in studies, so don't panic. Still, before combining them, please check with your pharmacist or oncology team so they can keep an eye on how you feel.

Cannabidiol may increase levels of drugs that are metabolized by CYP3A4.
In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the key thing to know: your body relies on a liver enzyme called CYP3A4 to clear everolimus from your system. CBD can slow that enzyme down, which means everolimus can build up higher than expected. We've actually seen this happen in people, not just in lab dishes, so this is a real concern, not just a theory.

When everolimus levels climb, the side effects tend to climb too. That can mean more mouth sores, low blood counts, infections, high blood sugar, or lung irritation. Everolimus is a drug where dosing has to be precise, so even a modest bump can matter. Please don't start or stop anything on your own. Talk with your oncologist or pharmacist first, since they may want to check your levels or adjust your dose if you use CBD.

Cannabidiol may increase everolimus levels.
Everolimus is a substrate of CYP3A4 enzymes. Cannabidiol has been shown to inhibit CYP3A4. A very small open-label clinical study shows that cannabidiol 12.5 mg/kg twice daily increases the area under the curve of a single dose of everolimus by about 2.5-fold. A retrospective study in children taking everolimus for tuberous sclerosis has found that adding treatment with cannabidiol increases everolimus serum concentration by a median of 9.8 ng/mL. In a case report, a 6-year-old girl stable on everolimus with refractory tonic seizures was started on cannabidiol titrated up to 200 mg daily for 6 weeks. This led to elevated levels of everolimus.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to clear everolimus out of your system. CBD can slow that enzyme down, so the everolimus doesn't get broken down as quickly and can build up in your blood. This isn't just theory either. A small study showed CBD raised everolimus exposure by about 2.5 times, and real-world cases in kids have shown the same climb.

About Everolimus

What Is Everolimus Used For?

Brand names: Certican, Afinitor, Zortress, Afinitor Disperz

Everolimus is used to treat several serious conditions. In cancer, it is prescribed for advanced hormone receptor-positive, HER2-negative breast cancer (in postmenopausal women), certain neuroendocrine tumors of the pancreas, GI tract, or lung, and advanced renal cell carcinoma (kidney cancer) when other treatments have stopped working.

It is also used for people with tuberous sclerosis complex (TSC) — a genetic condition that causes benign tumors to grow throughout the body. In TSC, it treats kidney tumors called angiomyolipomas, brain tumors called SEGA, and seizures. In kidney transplant patients, it helps prevent the body from rejecting the new organ.
