CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Formoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cannabidiol (cbd) interacts with.
In vitro research shows that cannabidiol inhibits CYP2D6. Theoretically, concomitant use of cannabidiol with CYP2D6 substrates might increase the risk for adverse effects from these substrates. However, a clinical crossover trial in healthy adults shows that cannabidiol does not inhibit dextromethorphan, a substrate of CYP2D6.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version on pairing CBD with your formoterol inhaler. Your body partly uses an enzyme called CYP2D6 to break down formoterol. In lab (test tube) studies, CBD can slow that enzyme down. In theory, that could let a little more formoterol linger in your system, which might nudge up the usual side effects of this kind of medicine, like a faster or pounding heartbeat, shakiness or tremor in your hands, jitteriness, or a headache.
I want to be honest that this is mostly a theoretical concern. A human study found CBD did not actually block the breakdown of another CYP2D6 drug, so real-world impact may be small. Formoterol is also inhaled, so very little reaches the bloodstream. Still, if you notice a racing heart or extra shakiness, check in with your pharmacist or doctor.

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

Research shows that cannabidiol inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabidiol with CYP2C19 substrates may increase the risk for adverse effects from these substrates.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to keep in mind: our database lists Formoterol under drugs cleared by the liver enzyme CYP2C19, and CBD can slow that enzyme down. In theory, that could let a little more Formoterol hang around in your body, nudging up its usual side effects like a fast or pounding heartbeat, shakiness or tremor, nervousness, or muscle cramps.
That said, I want to be honest with you. Formoterol is an inhaler that mostly acts right in your lungs, and CYP2C19 is not its main route of breakdown, so this overlap is more of a caution than a sure problem. If you use CBD and notice your heart racing or extra jitters, mention it to your pharmacist or doctor before changing anything.

When that pathway gets blocked, a little more formoterol can linger in your system than usual. Formoterol is an active drug (not a prodrug), so more of it means a stronger effect, including the side effects beta agonists are known for, like a faster or pounding heartbeat, jitteriness or shakiness, a headache, or muscle cramps.
Now, a practical point: formoterol is inhaled, so very little reaches the liver to begin with, which softens this concern. Still, if you notice your heart racing or feel more wired than usual after adding CBD, mention it to your pharmacist or doctor before changing anything.

Formoterol fumarate inhalation solution (nebulizer; sold as Perforomist and as generic formoterol fumarate) is used for long-term, twice-daily maintenance treatment of bronchoconstriction (tightening of the airways) in people with chronic obstructive pulmonary disease (COPD). This includes chronic bronchitis and emphysema.
It is not for sudden COPD flare-ups and not for asthma.
