This blood pressure medicine is cleared by an enzyme called CYP3A4, and grapefruit blocks that enzyme, so drug levels can climb and blood pressure can fall further than intended. It is one of 11 major-rated interactions among the 239 supplements and herbs on this list, and that count is high enough that I would read carefully here rather than skim.
Most of the 187 moderate entries either add to isradipine's pressure-lowering effect or push against it, so control can drift in either direction. St. John's wort does the reverse of grapefruit, speeding up metabolism and weakening the drug, and a smaller group of 13 ingredients can affect heart rhythm.
