Isradipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the gist for Isradipine: your liver uses an enzyme called CYP3A4 to break this blood pressure pill down. Lab studies (done in test tubes, not in people) suggest that Terminalia arjuna may slow that same enzyme. If it does, less isradipine gets cleared and more stays in your system, which could nudge its blood pressure lowering effect a bit too far.

In real terms, that might mean dizziness, lightheadedness, flushing, headache, swollen ankles, or a faster heartbeat. I want to be honest though, this is theoretical and based only on lab work, so it may not happen at all. Don't stop either one on your own. Just give your pharmacist or doctor a heads-up before combining them, especially if you notice your readings dropping.
