Why this applies to Isradipine

Isradipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist: your blood pressure medicine, isradipine, is cleared from your body by a liver enzyme called CYP3A4. In lab dishes, plumbagin (a compound in Ceylon Leadwort) appears to slow that same enzyme down. If that were to happen in real life, isradipine could build up a bit higher than expected, which might mean more of its blood-pressure-lowering action, plus side effects like dizziness, flushing, headache, a faster heartbeat, or swollen ankles.

The reassuring part: this is only a theoretical concern based on test-tube work, never actually reported in people. Still, since isradipine already drops your blood pressure, it's worth a quick chat with your pharmacist or doctor before combining the two.
