Why this applies to Telmisartan, Amlodipine

Telmisartan, Amlodipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cubebs interacts with.

Several compounds isolated from cubebs can inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this effect has not been reported in humans. Theoretically, cubebs might increase the effects and side effects of drugs metabolized by CYP3A4. These include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), triazolam (Halcion), and numerous others.

Your Twynsta combines two blood pressure medicines. The amlodipine half is processed in your body by a liver enzyme called CYP3A4. Lab studies suggest certain compounds in cubeb berries may slow that enzyme down. If that happened in real life, more amlodipine could linger in your system, which might make its effects (and side effects) a little stronger. Think along the lines of swelling in the ankles, flushing, dizziness, headache, or blood pressure dropping lower than expected.

The good news: this is only a theoretical concern from test-tube research, not something seen in people. A sprinkle of cubeb as a spice is very unlikely to matter. Concentrated cubeb extracts or oils are the bigger question, so check with your pharmacist or doctor before using those.
